When you need it
- Any competitive acquisition where the solicitation requires a Past Performance Volume (Section L) or asks for past performance information as part of the proposal.
- Mandatory when the solicitation cites FAR 15.305 or includes a clause requiring offerors to provide past‑performance data for each contract listed.
How to fill it in
- Identify the most relevant contract to the current solicitation’s requirements.
- Obtain the following from your records or the contracting officer’s archives:
- Customer & Agency name and Point of Contact (POC) – include name, title, phone, email.
- Contract value (total dollar amount awarded).
- NAICS code(s) used for the award.
- Period of Performance – start and end dates.
- Relevance mapping – brief statement linking the contract to a requirement or evaluation factor in the solicitation — see relevant section.
- Verify that your company has an active SAM registration and up‑to‑date representations & certifications (FAR 52.204‑8)【S17】 before submitting.
