When you need it
Required at the submission stage for most public contracts, especially when the contract notice asks for a "letter of transmittal" or similar. It is usually mandatory in the tender pack's submission checklist.
How to fill it in
- Insert the contracting authority’s name and reference number.
- State your company name, address, and contact details.
- List the tender title and reference (as given in the call for competition).
- Confirm that all required documents (ESPD, technical specifications, price schedule) are attached.
- Declare that you meet the eligibility and selection criteria set out in Article 57 and Article 58 of Directive 2014/24/EU.
- Sign and date the letter.
