When you need it

Use this template during the pre‑tender stage for any procurement above the thresholds set out in Article 4 of the Directive (e.g., EUR 134 000 for central authorities). It is mandatory for contracts that fall within the scope of the EU public procurement rules.

How to fill it in

  1. Identify the applicable threshold (Article 4) and confirm the contract value exceeds it.
  2. Determine which professional or trade registers apply to your business:
  3. For all suppliers and service providers, cite the Registro della Camera di commercio, industria, agricoltura e artigianato.
  4. For supplies and services also list the Registro delle commissioni provinciali per l'artigianato.
  5. If you provide professional services, add the Consiglio nazionale degli ordini professionali.
  6. For works or services contracts include the Albo nazionale dei gestori ambientali where relevant.
  7. Obtain a recent extract (no older than 12 months) from each required register and attach it to your tender file.
  8. Record the registration numbers, issuing authority, and date of issue in the template fields provided.
  9. Verify that any certificates cited are still valid on the submission deadline.
